History: 30-year-old male, spell vs seizure in renal transplant patient

Findings

Symmetric calcifications in the caudate nuclei, putamen, thalami, and dentate nuclei.

This was stable compared to the comparison 2 years prior.

Chart review revealed a parathyroid hormone level of 242 pg/ml (normal range 18 - 80 pg/mL) 4 years prior,  just before his renal transplant.

Impression

Fahr syndrome secondary to secondary renal hyperparathyroidism.
